Ginkgo biloba: separating the myth from the evidence
Few supplements carry as much baggage as Ginkgo biloba. To some it is the original brain herb, sharpening memory and warding off decline; to others it is a debunked relic that big trials proved useless. Both camps are partly right and mostly wrong, because they are usually arguing about different questions. Ginkgo genuinely is one of the most-studied botanicals on earth — which means we can actually be specific about what it does, what it doesn't, and why the dose and the form decide which story you get.
A living fossil
The ginkgo tree is astonishing in its own right — a "living fossil" essentially unchanged for over 200 million years, individual trees living a thousand years or more. Extracts of its fan-shaped leaves have been used in traditional Chinese medicine for centuries. The compound of interest is not the raw leaf but a concentrated, purified extract, and here the details matter more than for almost any other herb.
Why "standardised extract" is the whole ballgame
Nearly all the credible ginkgo research uses one specific standardised extract, known as EGb 761, defined by a fixed chemical profile.
That profile — roughly 24% flavone glycosides and 6% terpene lactones (the ginkgolides and bilobalide) — is what a trial means when it says "ginkgo."1 A bottle of unstandardised "ginkgo leaf powder" may contain a fraction of those actives, or an inconsistent amount batch to batch. So before asking "does ginkgo work?", the honest first question is "which ginkgo, at what dose?" — because the answer changes completely.
What it is proposed to do
Ginkgo's mechanisms are reasonably well characterised, and they cluster around circulation and protection.
It is thought to support blood flow, including to the brain; to act as an antioxidant through its flavonoids; and to reduce the clumping of platelets via its ginkgolides, which antagonise a signalling molecule called platelet-activating factor.1 That last mechanism is worth flagging early because it is double-edged: the same mild blood-thinning that forms part of ginkgo's rationale is also the source of its most important safety caution, below.
What the research actually shows — the honest version
This is where ginkgo earns its reputation for controversy, and where being straight matters most.
The single most important thing to know: the large, well-run trials that asked whether ginkgo prevents dementia in healthy older people came back negative. The US "GEM" study followed more than 3,000 older adults for around six years and found ginkgo did not reduce the incidence of dementia;2 a companion analysis found it did not slow cognitive decline either.3 If anyone tells you ginkgo prevents Alzheimer's, the best evidence says otherwise. That is the myth, properly buried.
The more interesting, more positive signal is a different question: ginkgo's effect in people who already have some cognitive impairment, at an adequate dose. Here the picture is modestly favourable — earlier controlled trials reported benefits in dementia,4 and a systematic review and meta-analysis concluded that at a dose of around 240 mg/day of the standardised extract, ginkgo produced small improvements in cognition and daily function in cognitive impairment and dementia.5 A Cochrane review was more cautious, calling the overall evidence inconsistent and unreliable — a fair reflection of a genuinely mixed literature.6 And there is some evidence for modest memory effects in healthy middle-aged and older adults using the standardised extract.7 The realistic verdict: a modest, dose-dependent circulatory-cognitive support — not a preventive, not a miracle.
How it is taken
The doses used in the more positive trials are typically 120–240 mg/day of a standardised extract, often split across the day, and — as with the adaptogens — taken consistently over weeks to months rather than judged acutely. Dose and standardisation are not optional details here; they are the difference between the extract that was studied and a leaf powder that wasn't.
Is it safe?
Ginkgo is generally well tolerated, with side effects usually limited to mild headache or stomach upset. But its blood-thinning property makes one caution genuinely important: ginkgo can increase bleeding risk, so it should be avoided alongside anticoagulant or antiplatelet medication (such as warfarin or aspirin), stopped before surgery, and used cautiously by anyone with a bleeding disorder. Ginkgo appears in Nooceptin at 100 mg of standardised extract, as one contributor to that formula's circulation-and-cognition side rather than as a standalone high-dose ginkgo product — the studied efficacy doses for ginkgo alone sit higher, at 120–240 mg.
