The Omega-3 Index: the number researchers actually use

Ask someone how much omega-3 they take and you will get a milligram figure. Ask what their Omega-3 Index is and you will usually get a blank look — even though it is the number that best reflects your long-term omega-3 status, and the one the research community has quietly standardised around. If you only remember one omega-3 concept, this is the one worth keeping.

Milligrams measure the input. The index measures you.

The dose on a bottle tells you what goes in. It says nothing about how much you absorb, how much you already get from food, or where your levels actually sit. Two people taking the identical capsule can end up in very different places, because absorption, diet and body size all get a vote. The Omega-3 Index skips all of that guesswork by measuring the outcome directly.

Specifically, it is the percentage of EPA and DHA — the two long-chain marine omega-3s — in the membranes of your red blood cells. If 8% of the fatty acids packed into those membranes are EPA and DHA, your index is 8%. It is a proportion, not a dose, which is exactly what makes it comparable from one person to the next.1

Why a red blood cell?

Because it has a memory. Red cells live for around three to four months, and their membranes turn over slowly, so their fatty-acid make-up reflects your average intake over months rather than whatever you ate yesterday. That stability is the whole point — it is the omega-3 equivalent of HbA1c, the blood-sugar marker that reveals the long-run trend a single glucose reading would hide.2 A one-off fish supper barely moves it; a sustained habit moves it a lot.

Practically, it is measured from a single drop of dried blood on a card, which you can collect at home and post to a lab. No fasting, no clinic visit.

The scale, and the 8% line

Researchers group the index into rough zones, and the boundaries are where it gets interesting.

Across large population studies, people whose index sits in the 8–12% range tend to have the lowest cardiovascular risk, while those down around 4% cluster at the higher-risk end.1,3 A 2021 pooled analysis of seventeen prospective cohorts — following thousands of people over time — found that a higher Omega-3 Index was associated with a lower risk of death from all causes, one of the more robust datasets in the whole field.4 None of this proves that raising your index changes your fate; these are associations, not guarantees. But they are unusually consistent associations, which is precisely why researchers reach for the index instead of the dose.

Most people are nowhere near it

Here is the uncomfortable part. A global survey of Omega-3 Index values found that much of the world — North America and northern Europe especially — sits in the low, 4–5% range, well below the zone associated with the lowest risk.5 Populations that eat a lot of oily fish, such as Japan and coastal Scandinavia, run much higher. For most Western readers, the honest starting assumption is that your index is low, and that closing the gap is what a supplement or a few more fish suppers is actually for.

It moves — but slowly

This is the fact supplement marketing skips: your index does not jump. Because it is built into cell membranes that turn over across months, raising it takes consistent intake over three to four months before it settles at a new, higher plateau. There is no loading dose that shortcuts the membrane.

The trajectory has a characteristic shape — a fast early climb that gradually flattens as your membranes reach a new steady state. How high that plateau sits depends on your dose, the chemical form you take and how well you absorb it. Miss doses and it drifts back down just as gradually. The single most important variable is not the size of any one dose; it is whether you keep taking it.

What actually raises it

The dose-response has been modelled carefully. The Walker 2019 analysis pooled controlled trials to predict how a given daily amount of EPA and DHA shifts the index from a given starting point6 — and it is the engine behind the projection in our own calculator.

Two things stand out. First, higher doses do reach higher plateaus, but with clear diminishing returns — the jump from 4.5% to 8% is far easier to buy than the last stretch toward 10%. Second, your starting point and how you absorb the oil matter as much as the number of milligrams, which is why a well-absorbed form taken with food does more per milligram than a cheap one swallowed dry. How to use this

You have two honest options. The precise one: order a dried-blood-spot Omega-3 Index test, take your reading, and re-test after three or four months of a consistent routine to see where you have landed. It is inexpensive and worth doing once, if only to replace guesswork with a number.
